Candidates need to have one year of experience in health assessments and active AR lic and preferably licensed in AR and OK.
Ideally this experience would include, Adult NP or a Family NP.
Our Nurse Practitioners help with all aspects of patient care, including health assessments, diagnosis, treatments and consultations. The majority of our business is related to in-home health assessments performed for Medicare Advantage Programs. Generally, our nurse practitioners perform health assessments and educate patients about preventative care and the prescribed treatments, particularly those with chronic conditions.
RESPONSIBILITIES
Perform health risk assessments in our patients home
Document any chronic conditions with appropriate diagnoses codes for HCC categories
Assess and document any situations that should be brought to the attention of patients Primary Care Physician
Direct Medical Assistant in administering any tests during the patient visit
Write orders for any follow up tests that need to be administered with results sent to patients Primary Care Physician
JOB REQUIREMENTS
Nurse Practitioners should be licensed within the state of the market area worked and should be approved and credential by Medicare.Estimated Salary: $20 to $28 per hour based on qualifications.
